Introducing In Home CareIn Home Care

In Home Care offers a comprehensive range of quality support services that enable older, frail people to remain independent in their homes.

Services can range from basic domestic assistance, to meeting the needs of older people with complex personal care and medical needs including dementia.

Packages designed to meet your needs

ACH Group provides services that respect and utilise the unique strengths, capabilites and cultural backgrounds of our clients. Our qualified and experienced caring staff coordinate a high quality personalised service.

Our staff are also skilled in providing services for people with dementia.

We always invite the older person to participate in their own care planning. We welcome and encourage support from family, friends and other advocates.

How much does In Home Care cost?

We are committed to providing a quality service regardless of your ability to contribute to the cost. Fees vary depending on the type of service provided. Service fees and policies can be discussed on request.

How to access the service

Anyone can make a referral to In Home Care services including -

  • General Practitioners
  • Hospitals
  • Service Providers
  • Self
  • Carer, relatives or friends.

Package entry requirements and fees

Community Aged Care Packages (CACPs)
Entry requirements: Meet ACAT* assessment requirements.
Fees: Weekly fee limited to a maximum of 17.5% of standard pension (reviewed annually with CPI*).

Rehabilitative Community Aged Care
Entry requirements: Meet eligibility requirements of CACPs. Favourable prognosis to rehabilitate within a 6 month time frame.
Fees: Weekly fee limited to a maximum of 17.5% of standard pension (reviewed annually with CPI*).

Extended Aged Care at Home (EACH)
Entry requirements: Meet ACAT* and RCS* assessment requirements.
Fees: Weekly fee limited to a maximum of 17.5% of standard pension (reviewed annually with CPI*).

Community Options
Entry requirements: Eligible for HACC* funded services.
Fees: Vary according to support provided and ability to pay.

Acute Transition Alliance (ATA)
Entry requirements: Referred by a public hospital for rehabilitation care and support.
Fees: Vary according to support provided and ability to pay.

Private Alternatives
Entry requirements: Services are available to any older person on a fee-for-service basis.
Fees: Fee schedule is available on request.

*ACAT: Aged Care Assessment Team. An assessment that determines the level of care required.
*RCS: Resident Classification Scale. Determines the level of funding that is available for an older person.
*HACC: Home and Community Care. Funding body made up of Commonwealth and State Government funds and is usually "matched" by the organization.
*CPI: Consumer Price Index.

Sunlife Exchange a Success!

During May a team of twelve ACH group staff enjoyed a ten day international cultural exchange to Nagano and Nagoya in Japan as part of a scholarship program with Japanese aged care provider Sun Life Group.
