Transition Care

Your care after leaving hospital is a critical time which requires support from health professionals and the right care.

At ACH Group, we provide transition care adopting a restorative and well-being approach and assist you in planning for your future care if further support is required. Care is provided in-home with an emphasis on restoration and rehabilitation that assists you in regaining independence.

Your transition care package, provided in-home after hospital, could include:

• Allied health support
• Nursing support
• Personal care
• Medication
• Care planning and other supports
• Short-term equipment hire

Transition care is available by referral through the hospital, so please speak to your doctor or Transition Care coordinator.

Transition care is also a part of our residential care accommodation.
